Q: Do I need a doctor to get a blood test through The Medical Foundation?
A: The Medical Foundation allows greater participation in one's own healthcare. Yes, your healthcare provider can refer you to our lab, but it’s not a requirement.

Q: Do I need to get approval from my health insurance before ordering a test?
A: We do not accept Medicaid, Medicare or insurance for Direct Testing. You must pay with Visa, Mastercard or debit card at the time the test is ordered to receive the special price. If you prefer to bill insurance, please come to one of our outpatient locations.
Q: Do I need to make an appointment?
A: You do not need to make an appointment. Our hours and locations are listed here.
Q: Do you offer interpretation of the blood test results?
A: No. The test results will indicate whether your results are within normal or abnormal ranges. You will need to consult your physician for any further interpretation.
Q: How do I order a test?
A: It’s simple! Just find the test you would like to order and walk into any of our locations and order the test. You will need to pay for the test before you have it completed and you’ll be able to look at your results online.
Q: What if I don’t have access to view my results online?
A: You may pick up your results at any of our locations by presenting a valid photo ID.
Q: What type of payments will you accept for my testing?
A: We will accept Visa, Mastercard, or debit cards
Q: What do I need to do to prepare for my test?
A: Under the testing description you will find a space that says “Fasting” or “Patient Prep.” Follow the guidelines presented in this area.
Q: How do I view my test results online?
A: When you register for your test, a Medical Foundation employee will give you a brochure describing how to access your test results. If you have trouble finding the results, please email firstname.lastname@example.org or call 574.236.7263.
Q: How does The Medical Foundation protect my privacy?
A: Access our HIPPA and Privacy Policies.
Q: Can I take my results to my doctor?
A: Yes. You will have full access online to your test results and will be able to print as many copies as you like. Share your results with whomever you feel comfortable.
Q: What do I do if my results are abnormal?
A: The Medical Foundation encourages anyone with abnormal results to contact their family physician for further care.
Q: Is there an age limit on who can order tests?
A: Yes. Minors under the age of 18 will need to have a parent present at the time the lab work is completed. Please have state issued identification with you when you come to have your test completed. The Medical Foundation reserves the right to ask for identification before testing is performed.
